Breast reconstruction is a surgical technique that gives a form to your breast following a mastectomy, that is a surgery that removes your breast to treat or prevent cancer. Flap based breast reconstruction surgery is a procedure in which tissue from one part of your body, usually your abdomen, is transplanted to create a new breast mound. Flap based breast reconstruction is a complicated operation. A significant portion of breast creation using your own body tissue can be completed during your mastectomy (immediate reconstruction), although it may also be done as a second procedure later on (delayed reconstruction).

About Breast Reconstruction Surgery (DIEP)

During a deep inferior epigastric perforator (DIEP) flap operation, Mr Ammar Allouni will remove a portion of your abdominal skin and fat, as well as any blood vessels associated with it. The blood vessels are connected to those in your chest using complex microsurgical techniques, and the skin and fat are used to construct a breast.

Breast Reconstruction surgery is performed under general anaesthetic depending upon the type of surgery and usually spans between 5 to 6 hours. Typically, there are two main types of Breast Reconstruction surgeries.

Implant based breast reconstruction: In this type of breast reconstruction, plastic or saline implants are used to rebuild breast tissue. Sometimes, a combination of implants and body tissues are used to recreate the breasts.

Flap based breast reconstruction: In this type of breast reconstruction, generally the autologous tissues from the abdomen, thigh, back or bottom are taken from your body and utilised to create breasts. So, this is also referred to as autologous breast reconstruction surgery.

There are four main types of Flap based breast reconstruction surgeries. 

Latissimus dorsi (LD) flap: In this technique the tissues and muscles are removed from the back. Then the LD flap which is still linked to its own blood supply, is transplanted to the breast area through the chest.

TRAM flap: In this technique the skin, fat, blood vessels and muscles are removed from the abdomen and transferred on the breast as free flap.

DIEP flap: In this technique only the skin, fat and blood vessels are removed from the abdomen and minimal muscle tissue is taken to form the breast mound. Then Mr Ammar Allouni reconnects the blood vessels in the breasts with his immense surgery mettle.

TUG flap: In this technique the tissues and muscles are taken from the inner and back thigh and transplanted to form the breasts.

During your first appointment at our clinic, Mr Ammar Allouni will clarify all your questions regarding the procedure. He will recommend surgery only if it is the best option for you. You will also be encouraged to clarify any doubts regarding the recovery period, post-surgical care and the expected outcomes.

He will take your detailed medical history and conduct relevant clinical assessments, if surgery is indicated in your case. Additionally, he will inform you of the potential risks and complications of the surgery.

Some of your medications might require adjustments prior to the surgery or additional drugs may be prescribed. You will be asked to discontinue drugs like aspirin and herbal medications (if any) for a few days before and after the procedure.

Mr Ammar Allouni will also inform you about certain precautions to be taken before and after surgery. He will instruct you to quit smoking and to avoid consuming alcohol and recreational drugs for a few days before and after the surgery.

All surgical procedures leave scars, and the location and size of scars after breast reconstruction surgery are determined by the technique used. However, the scars will fade gradually.

It is difficult to predict exactly how a reconstructed breast will appear and feel in the future because it depends on case to case basis. You must also set reasonable and realistic expectations of the likely result. Potential risks of Breast Reconstruction Surgery (DIEP) include blood clots, asymmetrical breasts, necrosis after DIEP, implant ruptures, weakness, pain or sensitivity issue at the donor site, infection and bleeding.
